  • This application MUST be filed out in its entirety and signed any place a signature is required.
  • A resume' will not be accepted in lieu of an application. Resume's may be attached to the completed application.
  • Applications can be returned to the 911 Center at 912 Spring Creek Rd, Bainbridge or to the Decatur County Commissioners Office at 203 W Broughton St, Bainbridge.

Open Records Request Form:    

  • Open records requests are accepted Monday - Friday, 8:00 a.m. - 5:00 p.m.
  • Open records request forms can be faxed to 229-248-3857 or may be returned to the 911 Center at 912 Spring Creek Rd, Bainbridge.
  • You will be contacted by the 911 Director within 3 business days to verify if the records are available for release and when they will be ready for pickup.
  • Records Requests will be picked up from the 911 Center at 912 Spring Creek Rd, Bainbridge.
  • Payment for records requests will be due at the time the record is picked up. Cash or money order only will be accepted and cash must be correct change.
